Here are some explanations for these specific items:
Iceborn Gauntlet:
The Iceborn Gauntlet allows you to slow enemies, gives you armor, cooldown reduction and bonus damage for each Q, which can give nice chunks of bonus damage, while still giving you tankiness.
Mercury's Treads:
Everyone needs boots. These boots, however, give you tenacity, which means that all CC on you will be reduced in time, so that it's easier for you to remain tanking, chasing or fleeing. Also gives nice MR, which is good, since most magic damage is burst damage.
Spirit Visage:
It gives health, MR, cooldown reduction AND additional lifesteal to both your passive and your Bloodthirster.
Randuin's Omen:
A nice slow active, some armor and some health. It's also quite cheap!
Bloodthirster:
This item gives 20% lifesteal. This basically doubles your current lifesteal, allowing you to heal back huge chunks of your HP with only 1 Q, or even just autoattacks. With Spirit Visage, Passive and BT, you have 48% lifesteal.
Black Cleaver:
Your Q is around 90% of your damage. This is Physical damage and therefore it's quite useful to have armor penetration, seeing as armor chunks your damage greatly. It also gives a little bit of tankyness and some cooldown reduction, allowing your to spam Q even more!
This build has worked great for me, so far. The slight lack of hp is made up for by the incredible amount of lifesteal, the good amounts of MR and armor and, of course, the damage.

Really, this is just to your own preferral. The build works fine with ghost aswell, but i personally prefer flash, because ghost is too situational. Flash can be used in any part, to both chase or flee. It can be used as a nice escape or opportunity to finish someone off in the lane, aswell as the jungle. Ghost only seems to be quite strong in lane or in the river, because you can't flash over walls with ghost... The name says it. Feel free to use ghost, though! It just seemed quite unnecessary to me to use ghost when you already have both the slows from W and Iceborn Gauntlet!
The teleport is just the most important spell for Nasus, which is self-explanitory, so i'll just skip that subject.

With Nasus, you can apply different playstyles, such as freezing the lane, trying to exchange a lot, sacrificing health to lasthit, or even pushing, whilst warding extremely well. All these playstyles have their pros and cons. Let's recap.
Freezing the lane:
Pros:
- Hard for enemies to gank you/kill you, whilst easier for your team
- Bigger chance of recieving optimal XP amounts
Cons:
- The enemy might get a more dominant position over you
- It's quite boring, sometimes
Exchange a lot:
Pros:
- A chance of getting total lane-domination
- Might allow you to stack more
Cons:
- Increased risk to lose lane
- Increased chance of recieving good ganks from enemies
Sacrificing health to lasthit:
Pro:
- Optimal amount of stacks
Cons:
- Very high risk of enemies taking a very dominant position over you or to kill you
- If done at the wrong time, you're sure to die in a gank
Pushing with wards:
Pros:
- Chance to acquire higher lane dominance
- Works well to stack
Cons:
- Lots of money will be spent on wards
- Chance of meeting other enemies whilst warding
- You have to watch the minimap constantly
These are the main pros and cons that i've found for these playstyles. Some work better against certain champs, while others might not. Pick the one(s) that seem most suiting for the current situation and go with that. If your current playstyle doesn't work (anymore), swap to another one that does. Choosing the right playstyles per situation might take some practice, but will surely pay off!

I've chosen a slightly more aggressive buildtype than the usual one(s). It really DOES seem to work, though, because you are still quite tanky and actually able to lifesteal back a whole lot of HP in teamfights, or even 1v1's. Eventually you can take on 2v1's or even 3v1's while only being as fed as each of the enemies you are fighting.
