Here's a sample About the Author section. It's just a small, simplistic section that you can use to tell a few things about yourself, your relationship with the subject you are writing about, and - don't forget - why you're the one who should be teaching the reader all of this.

Keep in mind though, you aren't Hilary Clinton who is writing her memoirs as a boost to her campaign for 2016. Keep this short and concise. People did not come from far and wide to read a biography about you. They came to take knowledge on the subject you're writing about.

Introduction
The Introduction is where you give a brief summary of the champion (or other subject) your guide is about. Do not go in-depth here; most champions can be described in one or two sentences. However, you can take a brief look at their skills and what they do. An example from my Zed Guide would be:
" Zed excels at laning, split-pushing and roaming due to his high dueling potential, exceptional wave clear and superb mobility. Both Razor Shuriken and Shadow Slash are area-of-effect physical damage abilities - with a relatively low cool down - which allows him to clear creep waves with exceptional speed."

Here you give a brief overview of the champion's strengths and advantages. Personally I really dislike when people only have a list of pros and cons without explaining why those are a reality. Some are fairly obvious, others aren't. Just describe why you would pick the champion and in which situations they would flourish.

Every champion has weaknesses, so it's best to explain them. Especially when a champion has such flaws that they simply should not be picked in certain situations. Jarvan IV is not one of them, but mentioning that he is very dependent on hitting that infamous

Jarvan IV's Skillset

Martial Cadence |
(Innate): Jarvan IV's first attack on a target deals 10% of their current health as bonus physical damage (caps at 400 damage).
This effect cannot occur on the same target more than once every 10 / 8 / 6 seconds.

Dragon Strike |
(Active): Jarvan IV extends his lance in a straight line, dealing physical damage and lowering the armor of all enemies hit for 3 seconds.
If the lance connects with a deployed Demacian Standard, it will pull Jarvan IV to it, knocking up enemies within 180 range along his path for 0.75 seconds.

Golden Aegis |
(Active): Jarvan IV slows enemies within range for 2 seconds and shields himself for up to 5 seconds. The shield gains strength for each enemy champion within range upon activation.

Demacian Standard |
(Passive): Permanently grants Jarvan IV bonus armor and attack speed.
(Active): Jarvan IV throws a Demacian flag to the target location, dealing magic damage to enemies within 75 range. The flag will remain for 8 seconds, providing an aura that increases the attack speed of allies within a radius of 600, also grants sight within 700 range.

Cataclysm |
(Active): Jarvan IV leaps to an enemy champion, dealing physical damage to it and creating a 325-radius circle of impassable terrain around themselves. This circle lasts for 3.5 seconds and also grants sight within 825 range.
Jarvan IV can prematurely destroy the impassable terrain by activating this ability again.

Alright, two big chapters. First, something regarding the Skillset Chapter; don't do the actual skill descriptions, nor copy the descriptions from LoLWiki like I did here (Just did that as filler). The reason for this is that we already have the tooltips on MOBAFire, so you might as well give your own description which explains what the skill does, how you use it, and why it is strong or weak.
Secondly, there is the skill sequence. As most, I have decided to include a skilling priority, which indicates which skill should get a skill point if possible. It is a personal feature really, if you dislike it; then simply take it out and I am sure no one would complain. Anyway, right here you can give a concise explanation on why skill X is maxed first and why others are not. If your skill sequence deviates from the standard, compare it to that as well.

Starting ItemsNow here's a chapter where you can just start explaining your item choices, as given in the Quick Guide. Simply go through them, explain why the item is good on them, but above all else, explain why it is better than any of the alternatives. Seriously, so many authors miss that very basic idea so often and do not cover it: Doran's Blade sure is great, but wouldn't Long Sword be better?.
Just make sure people don't have to ask that.

On Guidewriting

The previous sections have been filled up with a Lorem ipsum text but I did want to touch general guide writing. It is important that you, as author, explain subjects in a concise way, but also really explain them. Most guides fail to construct a proper Laning and Teamfighting sections.
Basically just write your guide and read it as a five-year old; if you end up asking "Why?" too often, then you should probably do a bit more explaining (Disclaimer: The author of this guide cannot be held responsible for possible consequences of this).

This Particular Section
|
This section is good to list general tips and tricks most people don't know about or are simply unaware of. Usually it's directly related to the champion you're writing a guide about but that doesn't necessarily have to be the case.
For clarity, you should fiddle around with colours, images, anything that could just make your reader understand that it is important. However, don't overdo it. Especially no animated stuff, that's just a big big nope.

The Cheat Sheet

Mostly because I haven't been able to really put it conveniently, but I do want to address the cheat sheet. Most authors don't pay attention to it at all, and end up with a very messy and unorganised cheat sheet. The cheat sheet is sort of the business card of your guide; if it looks awful and tough to read, then big chance not many people will bother to look at the guide. Yes the Quick Guide introduces a new way to represent the cheat sheet, but it's less vast and does not include notes.

I personally dislike matchup sections a whole lot. Mostly because you pretty much have to update them every single patch. Also, in 90% of the cases it's just going to be like "Just do the regular stuff.", which is pretty obvious.
