This should be added automatically in your guides and on your profile if you've verified your summoner name on the website. I believe you've done so in the past (your profile doesn't say you're "Unverified"), but for some reason it's not showing up. Could you try editing your profile and removing your summoner name (make sure to update) before adding it again and re-verifying?
If that doesn't do it, it probably means there's a bug which I'll have to contact the developers about.
