Great guide my friend, but allow me to make some sugestions:
1- Your voidling actually benifits from your bonus ad (no joke this is legit, so your 3+ ad will actually go towards your voidling =D). Just mention to be a cool kid if you wish.
2- There is a random bracker infront of null void zone is skill explination
3- When harassing I would use
4- Consider takeing an early dorans if you are fighting a lane sustain heavy champ like vlad or morg, and boots/pots if you are fighitng a harras or "fighty" mid like cass or leblank.
5- You say that you should pick up a QSS if they have exhaust. Not trying to be rude but unless they have a lot of CC or a bursty mid QSS isn't needed. I LOVE QSS TBH but if they just have exhuast I don't belive this justifies grabing it.
6- Consider expanding on your countering champs mid section. Curretnly I am working on my own, and it is tempting to leave short little explinations for each one, but realisticly it would be best to give a toatle overhall of what they can do =D.
Well that is all I have to say. A PM about this and if you make these changes/ prove that I am makeing a stupid arguement would be swag. A +1 will be otw if you do so=)

3. I don't understand this one. Are you trying to say it's better to use Malefic Visions first?
If so, that IS a good idea, but I feel that you should use Call of the Void first. This is because Malefic Visions has short range, and thus you are very predictable if you use Malefic visions first.
4. I see your point, but I just start boots +3pots because Malzahar has no escape. So I will need the extra movement to avoid junglers.

It is definately a good guide, but i feel you could be a bit more in-depth on your explanations, especially on your matchups chapter. Also, since you are taking flash, why not
