Seems weird but Teemo is the ultimate counter, his Q goes past your W therefore you can't do anything while he guns you down after. Your best bet is to get Legend Tenacity instead.

Champion Build Guide
If you consider that starting Doran's Shield still won't be enough to sustain Heavy Poke (i.e Teemo, Vayne, Quinn) then get Second Wind instead of Revitalize.
