Introduction
I used to play "pre-rework" Swain however I personally found after a few months of playing him. Old Swain got boring minutes after the start of the game. I got to the point where I only played Swain in ranked for wins.
Needless to say I was excited for the "new" Swain. Now that his rework has "gone live" I have been diligently working to come up with a new build for the sexy rework. After a few tests trying to maximize the combat healing/spell, steal/life-steal, well retaining a good amount of "tanking ability" and "damage output".
I knew this was a fun build when I 2v1'd a top Nasus(Same level) and jg Tryndamere (1 level below). (Both used their "R" and I tanked the damage & out sustained both champions ending in double kill).
A few side notes to this build:
Farm Your CS CASULS - your items will be the difference between being able to 1v1 top, or 2v1 "life-steal/heal" **** on a top laner and his "trash" jungle.
This build shines mid-late game, however "New Swain" can be a lane bully too.
Thanks and hope you enjoy the build. YOU WILL WIN ALMOST EVERY LEVEL 6 TRADE IN THE GAME BECAUSE OF YOU INSANE HEALING/LIFE STEAL. The passive rework on his ult is great, and the amount of max hp you can get with grasp, overgrowth and passive is crazy.

I buy the items in this order: (1) -> (2) ->(3) (Top -> Bottom)
The CP will give you sustain and mana so you can poke. You can also chug it at level 6 for trades to increase healing even more.

(1E)RUSH ROD SAVE MORELLO FOR AFTER YOUR DEFENSE ITEM!
- Rod of Ages -> Morellonomicon
- RoA gives More HP & Damage (I take this first for Eternity Passive: "Restore mana equal to
15% of damage taken from champions. Restore health equal to 20% of mana spent, up to 25
health per cast, while toggle abilities can heal for up to 25 per second."
- Morellonomicon More DMG/CDR and Mana(for spam of high CDR) Much like the RoA, The Unique
Passives of the Morellonomicon are what makes this item viable. The grievous wound to
punish champions that want to trade with you & 20% max mana return on kills or assists.

- Boots are up to you. (I take AP pen majority of the time.)
- Buy SV first if opponent is AP, Buy HG if opponent is AD
- The SV gives +30% to all healing & HP/MR.
- The Hourglass gives you DMG, Armor, works with your ultimate.

The last item is up to personal preference. Don't take the armor unless they have lots of AD easily countered. The best choice is usually Liandry's torment because you will be in the middle of most fights, health, magic dmg and magic pen, more damage the long you are in combat... Whats not to love...
You can also go for Warmog's if the team is struggling in fights or I feel "strong enough" or Hextech Gunblade if you want to snowball or max your life-steal if its not already enough..Additionally it will give you a reliable slow to hit your E.

Killer Combo / Full Send Combo: Start the fight with the (E -> W -> Q) Then use R and repeat abilities strategically. ALSO USE YOUR HOURGLASS! ITS DOESN'T CANCEL YOUR ULTIMATE!
Going against melees is easy. If they go in on you put your W ontop of yourself. They then have to chose to give you dmg, life steal from soul rip, and stacks on your ult to do any damage. Hit them with your E to keep them where you want them whether that is in melee range of you or to kite away. USE YOU Q AS MUCH AS POSSIBLE AND TRY TO HIT THE CHAMP WITH ALL THE BOLTS for max dmg and healing.
