Spirit Visage is used almost solely to increase the healing from your ultimate. At a 20% cost for regenerating 50% of your health, Sadism only gives you a 30% health heal. Spirit Visage effectively increases 50% to a 57.5% heal, or an effective 37.5% health heal. This is an effective 25% increase in healing.
In short, if you have 3000 health on Mundo, Spirit Visage is giving you an extra 750 health as a heal from your ultimate.
While other items may also increase the healing, note that Spirit Visage should be used in tandem with those other items, not by itself.

it takes 20% of your CURRENT health jebus, so if you used it when you were at full health it would be 30% effective heal.
if you were at 50% hp when you used it, the effective heal would be 40%.

All in all, it makes Mundo happy to do this.

Sure it is cheap, but items such as HoG and GB are even cheaper and do just as or even better of a job of giving health for dr. mundo. I'm just trying to clear the upheld notion that you need a SV in an "ideal" setup because as you can see, other items outshine it.

Regen from +15% effect on 11.75hp/sec (hp regen at lvl 6: 11.75*0.15= 1.76/5 sec
Regen from +15% effect on ult: 1112+250=1362*0.5=681*0.15= 102.15/12 sec
Total Regen Over 12 Seconds: 0.75*12+125+1.76/5*12+102.15= 427.97 reg/12 sec
Converted HP over 12 Seconds: 250+427.97= 677.97 HP
Cost: 1650 - 1 gold/sec
HP: 500

As you can see, without regarding other effects, Giant's Belt is well ahead in cost worth.
Spirit Visage provides 10% CD Reduction. 10% CD Reduction is rather negligible on DR. MUNDO.

--Second, it will take off less than half a second off your cleaver, which already has a short enoough CD.
--Third, the CD off of
--Fourth, you'll only have time to use one ult per fight anyways, so a 7.5 sec reduction is not a difference maker.
Spirit Visage gives 30 magic resistance. This makes up for some of the lack of HP, but only effective against a lot magic damage.
Heart of Gold gives 5 gold/10 sec, so for 2, you will get 1 gold/sec. That means in about 10 minutes, it will pay for one Heart of Gold.
Giant's Belt is cheapest in terms of pure converted HP.
With each increasing lvl of Dr. Mundo over lvl 6, Spirit Visage will give you an extra 6.9HP/12 sec + 0.11HP/5 sec, a grand total of 7.17 converted HP per level, not a big increase.
